Tibial Post Wear in Posterior-Stabilized Knee Replacements: A Study of Three Contemporary Designs

ثبت نشده
چکیده

Introduction The tibial post in posterior-stabilized (PS) knee implants has been identified as a potential source of wear debris. While most studies have been unable to link the amount of post wear to differences in design, a recent study found a significant difference in location of post wear between two very similar designs, the IB I and IB II, which had only subtle differences in post location [1]. Based on these results from designs with similar bearing surfaces, the question arises as to the influence of post design on other contemporary PS knee implants and how changes in post design and location made to improve function may also affect post damage. This study examines the tibial post damage in three contemporary PS total knee designs. We hypothesized that differences in the tibial post design between these implants correlate with different damage patterns.

A kinematic comparison of fixed- and mobile-bearing knee replacements.

Mobile-bearing posterior-stabilised knee replacements have been developed as an alternative to the standard fixed- and mobile-bearing designs. However, little is known about the in vivo kinematics of this new group of implants. We investigated 31 patients who had undergone a total knee replacement with a similar prosthetic design but with three different options: fixed-bearing posterior cruciat...

Total Knee Post-Cam Design Variations and Their Effects on Kinematics and Wear Patterns.

Post-cam designs for posterior-stabilized total knee arthroplasy (TKA) implants have evolved over the last 2 decades. These designs have evolved from symmetric post and cam to asymmetric designs that include anterior post interactions to affect a kinematic change in full extension. All design changes have consequences on the resulting femorotibial contact kinematics and, depending on the amount...

Fracture of a Polyethylene Tibial Post in a Scorpio Posterior-Stabilized Knee Prosthesis

We report the case of a polyethylene tibial post fracture in a 72-year-old woman 14 months after a Scorpio posterior-stabilized (PS) total knee arthroplasty. The polyethylene wear was found around the fracture site of the post, especially over the anterior aspect of the post base. The failure mechanism of the post fracture in the present case was anterior impingement with excessive wear over th...

Evaluation of impingement of the anterior tibial post during gait in a posteriorly-stabilised total knee replacement.

Mechanical failure because of wear or fracture of the polyethylene tibial post in posteriorly-stabilised total knee replacements has been extensively described. In this study of 12 patients with a clinically and radiologically successful NexGen LPS posteriorly-stabilised prosthesis impingement of the anterior tibial post was evaluated in vivo in three dimensions during gait using radiologically...

Effects of Tibial Insert Slope on Polyethylene Wear and Stress

INTRODUCTION: Both posterior stabilized (PS) and cruciate retaining (CR) total knee replacement systems have demonstrated high rates of survivorship, high clinical knee scores and high patient satisfaction scores for decades. However, CR total knees sometimes exhibit more varied kinematics and a smaller range of motion than similar PS knees. Many factors can cause this, but one important factor...
